$75 per golfer (includes golf and dinner).

• The course is nine holes and is a walking course. A limited number of carts are available for an additional charge.
• $18 per additional dinner guest (non-golfing), $7.50 per child under 12.
• Tee times starting at 4:30pm. Register early for early tee times.
• Soft cleats and collared shirts required.

To register by mail: Please make checks payable to One Small Voice Foundation. Include the names and phone numbers of your foursome and mail to One Small Voice Foundation, P. O. Box 644, Elmhurst, IL 60126.

Registration deadline has been extended to May 27, 2011. All golfers must be pre-paid and registered. If you do not have a foursome, but would like to golf, please tell us your level of play and we can place you in a foursome.

All proceeds benefit pediatric research for hydrocephalus being conducted by Children’s Memorial Chicago. The One Small Voice Foundation is a 501(c)3 organization and your donation is tax deductible.
